The Director of Finance for the City of El Cajon is hereby authorized to deduct from my salary, each pay period, such amounts as may now or hereafter be payable by me under this policy plan purchased through LEO Web Protect. In the event of a rate change, I authorize a corresponding change in the amount deducted from my earnings, effective in the next available pay period deduction. Said amount is to be paid to the El Cajon Police Officers’ Association. This authorization may be terminated by me at any time through the filing of a written notice to LEO Web Protect, with the Finance Department, and with the ECPOA President or designee. The ECPOA must be made aware of any changes and I hereby agree to be responsible for any cost incurred by the ECPOA as a result of any changes to dues, insurance coverage, or any other ECPOA related expense paid for on my behalf by the ECPOA. This deduction is to take effect immediately by the date of this signed document.

ECPOA does not endorse this product, assumes no responsibility for any of the promises or representation made herein, and disclaims any liability with respect to its member’s utilization of the product(s) offered through LEO Web Protect or its affiliates.

Upon submitting this registration, you agree to have $2.88 per pay period for continued service withdrawn from my earnings.
